  • Please prepare the Medical record(Doctor’s latest note, Patient's history, Clinical findings, Diagnostic test results including biopsy, Pre and Postoperative care, Patient's progress, and Current medication, Etc... in English) and your image file(CT, MRI, PET-CT, Ect...) as DICOM file.
  • It would be grateful if you could send us when we ask by e-mail or send a CD COPY through EMS.

Korean Cancer Center Hospital will collect and use personal data for the following purposes:

To send medical bills, certificates, and necessary things for tests and health screening
To allow us to maintain communication and contact with you when needed for transmission of notices and answers to complaints
To handle the response to an online consultation and provide any new service and event information
To prepare for new service development and personalized service
To collect information on consumer-endangering services and the environment in compliance

2. Required Information
Personal Information: Name, Nationality, Address, Contact Details, Date of Birth, Gender. Ect...

3. Right to refuse the consent and disadvantages in case of refusal
You may refuse to consent to the above-mentioned purposes and may withdraw your consent at any time after consent. However, we inform you that the personal information collected is the minimum basic information necessary for a better process. Your refusal to consent may cause limitations in receiving services at Korean Cancer Center Hospital.

4. Terms of personal data processing & Termination
Personal data is retained and used for as long as the client receives services from Korean Cancer Center Hospital. The Hospital will delete your personal data immediately upon withdrawal after completing treatment.
